Here’s the application form and easier ways to apply for the Subhadra Yojna to get the financial assistance from the Odisha government.
All female members of a family can apply either offline or online on the specific application form, to avail financial assistance owing to women empowerment.
How to apply Offline: To apply offline, the beneficiaries have to visit their nearest Anganwadi Centre, Block Office, Mo Seva Kendra, Jana Seva Kendra and fill their forms with necessary documents.
Online Mode: Similarly, the beneficiaries can also go to the Subhadra Portal and fill out the necessary form online and submit it by attaching the necessary documents as follows.
Documents required: 1. Date of Birth proof: 2. Aadhaar Card 3. Aadhaar-linked mobile number 4. Income Certificate of family head 5. Ration Card and 6. E-KYC
